Provider Note Requirements

A provider note should state:

“Due to health and/or mobility-related limitations, the individual is currently unable, or experiences substantial difficulty, attending congregate meal services and would benefit from participation in a home-delivered meal program to support health, nutrition, and independent living. Home-delivered meal services are recommended at this time.”
